I am writing this letter to formally notify you that you are on probation. On ___/___/_____ you were given a warning for your poor performance and were not meeting management expectations for your job description.

You are being placed on probation for three months effective the date of this notice. If your performance does not meet managementís expectations by the end of this Employee Probation period your employment will be terminated without further notice.
