The Company hereby agrees to provide the following specified maintenance service to the above listed Customer.

This Equipment Maintenance Agreement shall be valid for one year from the Commencement Date. This agreement will be renewable for one year increments at the prevailing market rate. However, either party has the right to terminate this agreement at the end of the original or at any renewal term by mailing an agreement termination notice to the opposing party with at least a thirty day notice before the designated termination date. The Company may terminate this agreement with a fifteen day notice of termination if the Customer fails to make timely payments.

The following payment schedule covers maintenance services for 12 months and starting on the above listed commencement date. The maintenance fee is to be paid in advance and will be paid in installments of:
